Paycheck stubs for previous 30 days for all working adults in household

Current bank statement

Copies of all financial assistance (Food Stamps, TANF, Child Support, SSI, Social Security, Unemployment, Worker’s comp, Disability, Retirement, etc…)

Driver’s license OR photo ID for all adults in household

Birth Certificate/Social Security card for each adult and child in household

Copy of current lease signed by tenant and landlord or residence verification form or report cards for all school age eligible children
